Waiver Policy

All participants—every adult and each child attending—must have a completed and signed waiver on file.

​

Please complete the waiver prior to your visit. A parent or legal guardian must complete and sign the waiver for any minors.

 

Each waiver is valid for one year from the date it is signed. After that time, a new waiver will be required!

• Home Address

• Phone Number

• Email Address

• Child’s name(s)
